Updates to EBNF to BNF document

Hi folks,

I think I have an outstanding action to update the EBNF to BNF document.
Upon reviewing the comments, I discovered that I’d already done one set
of updates. I’ve done another.

I haven’t been able to address all of the comments because I didn’t
understand some of them and because some of them expressed a general
unease about the prose that I’m not sure how to address.

It’s in the ‘misc’ directory and isn’t (yet) part of the formal
specification. My proposal at this stage is to accept the PR and publish
the document. It is certainly imperfect and perhaps incomplete, but
(IMHO) it’s better than nothing.

Perhaps someone else could take up the editing pencil for a next
version.
