Neil Soiffer just posted this to the MathML list www-math@w3.org but it might be of general interest to anyone trying to produce accessible HTML+MathML documents, so I'm re-posting here for information. While some details are particular to the DAISY context most of the guidelines apply equally to MathML usage in HTML. David DAISY just published an update to their "structure guidelines" for MathML. While DAISY is concerned about accessibility, the structure guidelines are very general and with real life examples from textbooks such as how to deal with elementary math and mix math and text.
