As mentioned by Simon Pieters in the WHATWG mailing list, the example just
before 4.3.1.1 doesn't work because in |onchange="calculate(form)"|, |form| is
null. This should be changed to
|onchange="calculate(document.forms.pricecalce)"| or the change handler should
be moved to the fieldset element (where this.form isn't null).
