Per https://www.w3.org/Bugs/Public/show_bug.cgi?id=24322, I have
reorganized the spec by object. This distributes the Extensions, Events,
and Algorithms sections into sections for MediaKeys, MediaKeySession, and
HTMLMediaElement Extensions.
