Markup languages are defined in technical specifications, which

equivalent, but in most cases, some conformance requirements can not

のようにありますので、conformance というのは仕様の conformance requirements [1] という部分にあるように、

1. Criteria that can be expressed in a DTD.
2. Criteria that cannot be expressed by a DTD, but can still be
checked by a machine.
3. Criteria that can only be checked by a human.

のすべてを満たしているということですね。

重ねて質問させてください。このうち、1番目は valid と同義で、例えば (X)HTML5 では特定の場所以外では & を &amp;
にしなければいけない、などを満たしているかということでしょうか? 3番目は例えば img 要素の alt 属性に "画像"
とかではなくて画像が無くても意味をなす説明文にしなさいということでしょうか? 2番目はちょっと例が思いつきません (何かありますか?)。

well-formed はつまり well-formed XML として定義されているもので、valid XHTML5 であるかに関わらず
RSS だろうが atom だろうが XML としてパースできるなら well-formed ということですね。
