Work on insert and delete actions

Hi Erik,

If you obtained the source bundle before now to work on the insert and 
delete actions, then please note that I have added a note to each of 
insert and delete because I noticed they were missing the note that 
appears in other actions to say that they have "special" deferred update 
behavior.  Of course, I noticed this while removing the word "special", 
which was one of your last call comments.

And if you haven't obtained the source bundle yet to work on insert and 
delete, then hey, no time like the present :-)

iPhone runs Safari, which supports JavaScript and Ajax with all the 
bells and whistles. This means that you can easily target the iPhone 
with XForms processors implemented with Ajax :-)

This of course means you need an internet connection (iPhone supports 
EDGE and Wi-Fi).
