We recently produced a reflowable EPUB from an InDesign file with Circular Software. It features 350 automatically placed role=“doc-pagebreak” markers and dozens more accessibility features including landmarks, nested TOC, ARIA roles, A11y metadata, image and table descriptions and heading hierarchy. It is a valid EPUB and has no ACE violations.

The book "What About Us? Global Perspectives on Redressing Religious Inequalities” published by the Institute of Development Studies is freely distributable with the Creative Commons Attribution 4.0 International (CC BY 4.0) license, more info here<https://www.ids.ac.uk/publications/what-about-us/>.
