Explanation of the many GitHub issues

Dear WG,

Sorry for flooding you all with profusion of GitHub issues. This was a one-time task (well, the second such one-time task) to load all of the issues listed only in the Conneg, Guidance and Profile ont docs into GitHub and link them. Now that it's done bulk generation won't happen again!

Each issue now has a note on its source so for instance, Issue 387 [1] says "This issue was created in the Conneg by Profile document<https://w3c.github.io/dxwg/conneg-by-ap/> and is listed in it..."

The expected process is now that each of the Conneg, Guidance and Profile ont docs' issues will be addressed in GitHub and, when answered with consensus, the result moved into the relevant doc.

We have not yet tidied Requirement by de-duplication or harmonising those in GitHub and the UCR so, for the moment, all Requirements for each doc are tagged in it, except that those in Profile Guidance are only those tagged 'plenary=-approved'. See the Issues Summary in the Profile ont doc [2] for the bulk listing. De-duplication etc. can now happen publicly since all individual issues are in GitHub.
