Post-TPAC updates on the PWP document

Dear all,

I have spent some time trying to re-write some sections of the PWP document, based on the TPAC discussions and comments, and also some private input from Tzviya and Markus. The particular things I have added/changed are:

- added notes on the file access issue v.a.v. Service Workers (hopefully Daniel will provide with some more specific wording)
- almost completely re-wrote the section on identifiers. I have folded in a minimal version of my presentation slides as some questions with pending options, made a clear difference between identifier and locator (cf Peter Linss' reaction on the discussion), etc. I have also tried to make a cleaner difference and separation on URI-s vs. URL-s
- the role of the manifest has been re-inforced as something, probably, inherently part of a PWP (and whose details may be for later)

What I did, in practice, is to open a separate branch on the github repo, and made a pull request[1]. If you go to [1] you can see the differences, although it is not easy to grasp it that way. But [2] gives a fully readable version of the whole.

It would be great if you could have a look at what is there, possibly either comment or simply edit that branch for simple things like English errors, or raise issues (the great thing about git(hub) is that if you commit into the same branch it will automatically 'update' the pull request). I would then merge the versions to update the official editors' draft.
