One exception is SVSes (Standard Variation Sequences) for Compatibility Ideographs. But use of IVSes needs a registered IVD as you pointed out. Another approach is to define or use a new or existing glyph set and to use a font supporting the glyph set. Typically, many traditional Japanese fonts implemented as CFF/CFF-2 OpenType fonts have supported one of the Adobe-Japan1-x glyph sets or its subset.
