A Secure Data Store Use Case: COVID-19 Health Report

I was inspired by the DID use cases document to present a focal SDS use
case in a way that highlights the coverage of real-world privacy concerns
such as those raised by EFF.

The health report use case also helps clarify the opportunities for
decentralization and personal agency as we develop secure data store
standards.
