Updated Claims Data Model document

I've made the final updates I planned to the document before the IG
meeting.  They are all minor:

- fix reference to the use cases document
- remove leftover instances of 'user-centric'
- modify abstract to use text from architecture doc that highlights
verifiable claims rather than identity.
