internal comments, citations and glossary

I received some internal comments that we need to address.
Our citations are inconsistent
also when citing a book we need to state the page range
there was a request that we compile our citations to end note and not by chapter.
I think we should have very clear directions on how to do citations in our main page of our wiki.
