The WG discussed this and agreed to add an example that declares the
xhtml namespace. HTML4 does not have namespaces. We will add a
reference to the XHTML 1.0 specification, which does.

The WG agrees and will reword so that its clear param is used with a pair
of attribute values, name and value.
