At last there is an updated version of this document [1] for review. There is not a lot to review as most of the work has been revising which comments had been addressed and which had not. This is compiled in a list of pending edits [2] which complements the changelog. there are still a number of difficult problems outstanding (marked "PENDING") many of which require further discussion.
