Incidentally: I have just committed a substantial change to the repo on security model. The only substantive change is a parameter is added to requestMIDIAccess() to denote when system exclusive support is requested; I've written a large amount of detail on why that matters in the security section in the spec. Please take a look. I think this resolves issue 3 to my personal satisfaction, but I'd like some review.
