APG: Code examples on GitHub Pages

Hi Jemma,

All code examples on the GitHub Pages version of the APG <http://w3c.github.io/aria/practices/aria-practices.html> have been updated. After a manual check it seems not all code examples are linked to <https://github.com/w3c/aria/issues/255>  so I have filed a bug for that.

I have validated the document with W3C Linkcheck <https://validator.w3.org/checklink> to make sure there are no broken links (assuming all code examples are linked to). Two examples—toolbar and splitter—have broken links, but neither of those exist in any branch I know of; I have filed a bug <https://github.com/w3c/aria/issues/254>.
