The reason we removed it was because the <content> element became
mandatory (so it made having a default superfluous). I'm happy to
again make the <content> element optional (and hence, the whole
config.xml document optional), but it will mean some significant
changes to the current spec which I won't get time to do this week to
meet the publication deadline.

I'm reluctant to add more draconian error handling into the spec.
Personally, I want to bring back the automated selection of index.html
and make the config.xml file optional. We should add this to the
agenda for the F2F.
