HTML Accessibility Review

Hi all,

I'm well on the way to completing the review. I've been reviewing the six identified PRs [0], plus all of the other commits that weren't discounted for being non-substatial. Here are some more interim results for your consideration. I have attached two files:

 * review-notes-for-PRs.html contains the notes for the six identified PRs [0] plus one more commit that implements changes along the lines of the PRs but was missed at the time.

 * review-parked.html contains a list of commits I've identified as being potentially important, out of the non-PR commits, but haven't had time to ascertain their relevance, nor fully review yet.

If anyone has time, feel free to:

 * Check the notes for the PR reviews.

 * Check out some of the "parked" commits to see if they're relevant to accessibility, and let me know.

So far I have found several accessibility related commits, but do not have concerns about them. I hope that you will find the level of detail useful.

Further: I have been making some notes on my experience with, and suggestions for, this process and when I send the final review notes, I'd like to share those with you. I have also been tweaking my review scripts [1] as I went.

In the meantime, please let me know if you have any thoughts along the lines above.
