Hold on, we do already have pseudo per-stream settings. Even if you don't call it a per-stream setting, each stream has it's own flow control window that's set at stream begin by SETTINGS_INITIAL_WINDOW_SIZE. Perhaps it would be better to say SETTINGS_INITIAL_MAX_FRAME_SIZE, but I hardly think keeping a second integer for each stream is "complex". Yes, it adds more state, but it is optional.
