I should back-up one step and give an example in streamed connections:

GET /Connection-UUID/Header-UUID(/XPath)... HTTP/1.0
or decompiled to:
GET /Connection-UUID/Header-UUID(/XPath)... XHTML/1.0

I'm glad you put "max-conns" in then we can pre-set the Connection-UUIDs 
http://www.ietf.org/id/draft-nottingham-http-browser-hints-01.txt with a 
POST context to the UA.
