I don't think Michael would feel slighted by my saying that the path
scheme was mostly my design.  It wouldn't have been completed and
implemented so quickly without him, however.  So 'co-creator' is more
correct.

Host would not necessarily apply for a URN.  What would the document
server do with the URN info anyway?  It should probably know the URN
already.  Multiple URNs might map to the same document, but the same
document should result either way.  The client should preserve the
original un-translated request, and proxies may want to do so too, but
I'm not sure the document server needs it.
