word-2000 option

The word-2000 option seems to do a pretty good job!  But there are two glitches:

1) Even if you select normal HTML (*not* XHTML) output, it puts out XHTML (has
the namespace, <empty-tag-format />).

2) When you request a DOCTYPE, it is *not* inserted, but a corresponding
namespace declaration *is*.  And with DOCTYPE set to 'auto', it seems to
interpret that as 'omit'.
