http://www.w3.org/TR/2006/PER-xml-names-20060614/#ns-decl says "must  
either be xmlns or begin xmlns:." How about making that "begin with  
xmlns:."?

It also says "Element names MUST NOT have the prefix xmlns." Here MUST NOT  
is not marked up like the other RFC2119 terms, was that intentional?

The errors with the examples have already been pointed out.
