semantics, annotation, annotation-xml in mathml 3.0

        5.2.1 The semantics element
        5.2.2 The annotation element
        5.2.3 The annotation-xml element

not only is the TOC missing visible <> or </>, so is the document. so there are 3, not just 1. this makes it hard for the reader to dermine whether or not the elements are void elements or not. please fix. thanks.
