[html401] "http-equiv" attribute

the "http-equiv" attribute is described. Given that this attribute was  
never implemented as described I suggest that the specification gets an  
erratum describing what user agents have implemented or that the  
specification simply obsoletes the attribute given that it can't be used  
as described.
