There is no visual difference in the appearance of the
combo boxes under the two different circumstances that
you outline.
As you point out some are designed to do something as
soon as you press 'cursor down'. 

To access this functionality using the keyboard.

Internet Explorer or Opera.
The user has to know about the (not widely known)
keyboard function 'Alt - Down Arrow' on Windows,
'Shift - Down Arrow' on Mac, to access this
functionality as intended.

Netscape.
This functionality can't be accessed using the
keyboard.
