Authoring practices on mixed content and unsafe redirects.

I've updated the authoring practice in [1], [2] somewhat:  [1] now
spells out a prohibition of mixed content with a SHOULD NOT; maybe
that should actually be a MUST NOT.

[2] now spells out definitions of unsafe redirect chains, and slaps
a MUST NOT on them.

Both of these changes are motivated by my hand-written notes from
our discussions at the face-to-face, so I don't have a good pointer
to any meeting minutes for them.
