Re: [streams] Merge ReadableByteStream into ReadableStream (#430)

The automatic buffer allocation feature discussed at https://github.com/whatwg/streams/pull/430#issuecomment-194606911 has been added. See the newly added test.

Before making the change for the feature, I made some refactoring changes. The changes are separated into small commits, so should be easy to review.
