issue-49 (xml naming) and BLD

I think we owe the reader's an editors note at the start of BLD section
4 saying something like:

    Editor's Note: the names of the XML elements in this serialization
    are still subject to change.  See Working Group Issue-49.

If there are other similar open issues (semantic metadata?  lists?) they
should be similarly noted.
