Action item about improving insert/delete examples

I do not necessarily believe that the examples for insert and delete need 
improvement.  Moreover, XForms 1.1 is now in last call, and this has not 
been raised. 

If someone believes they should be improved, then let that person raise it 
as a last call comment (or post last call comment) with more specificity 
and then let a working group member take the action item.
