At the end of 7.20.1 "break-after" add, This property has no effect when it appears on a descendant of fo:table-header or fo:table-footer. Similarly for 7.20.2 "break-before". This property has no effect when it appears on a descendant of fo:table-header or fo:table-footer. This does not take into account the corner-case of a break inside a table header when table-omit-header-at-break is true - i.e. a break inside a table header or footer that does not repeat is now disallowed. I have marked this issue as closed; if you feel the resolution is unsatisfactory please reopen it (and say why!)
