I am completely satisfied with this resolution, thank you. 

I would note that the term "outer package" is perhaps misleading, as I 
believe the package in question is outer for XOP but inner to the 
multipart/related in the case of MTOM.  I presume this is only a matter of 
the wording of your email, in which case no further followup is necessary. 
 In any case, I am as noted above completely satisfied with what I take to 
be the intent of the resolution.  Thank you!

Noah,

You raised an issue, give the number 447[1] with respect to how an
MTOM/XOP aware SOAP node will make the determination as to whether or
not to perform MTOM/XOP processing. The XMLP working group have decided
that such processing will be triggered by the media type of the outer
package, which in the MTOM case will be application/soap_xop+xml
