Namespaces 1.1 - 6.2 Namespace errors

In the first piece of example code in 6.2 it is erroneously claimed that 
elements are in the "HTML" (sic) namespace. On the contrary the namespace binding 
declaration seems to indicate XHTML.

Similar errors occur in the second and third code examples in 6.2.

In addition the notion of "by default" seems to be confused with the notion 
of "default namespace".
