xml-names use of Hn violates accessibility guidelines

The xml-names document misuses an H3 element for the
master title, presumably because of using it for
rendering effect, which is a violation of the web
accessibility guidelines.

html2ps creates a strange outline tree as a result.  Amaya's
table of contents is also confused, but the confusion is
less obvious.

Also, would it be possible to purge the spam from the
archive, and given the limited number of genuine articles,
index by year, not month.
