In "http://www.w3.org/TR/1999/REC-xml-names-19990114/"
the following seems to be an error:

  A.3 Expanded Element Types and Attribute Names

  [...]

  2 HTML:CLASS <ExpAName name="CLASS" ns=http://www.w3.org/TR/REC-html40 /> 

The last line ought to be:

  2 HTML:CLASS <ExpAName name="CLASS" ns="http://www.w3.org/TR/REC-html40" /> 
                                         ^                               ^
