Maybe the attached image makes some things clearer.

upper left:  The current spec with arbitrary length nonce
upper right: No Nonce, an un-encrypted IV
lower left:  Why the Encrypted IV is the same like a block-length-nonce and 
an arbitrary IV
lower right: No Nonce, only the encrypted IV
