Thank you Andreas, all your typos have been fixed in revision 1.16 with one 
caveat.

&xenc; is supposed to be an entity that is exanded by an XML processor and 
I use it as a short hand to refer to the XENC namespace properly. I've 
added the entity to the DOCTYPE where it should've been.
