I would like to raise the following new issue.

Section "Understand SOAP header blocks", 3rd paragraph.

"Mandatory SOAP header blocks are presumed to somehow modify the 
semantics of other SOAP header blocks or SOAP body elements."

   The sentence is ambiguous: is it referring to "the SOAP Body",
   to "SOAP body children element information item" or to a
   combination of both?
