I am using Antenna House XSL Formatter

XSL Formatter V3.4 MR3a for Windows (3,4,2006,0316) - Registered Version

XSL Formatter V4.3 MR5 for Windows (x64) (4,3,2008,1119) - Evaluation 
Version (supports numeric keep values)

I do not see any errors when I run this fo, I have not tried any other 
processors to date....

running your example through xep I'm getting too many errors?

What engine are you using Mark?

 (format
    (sequence [master-reference prefaceMaster]
      (flow
        [error] com.renderx.xep.cmp.NoPageMasterException: state:
first filled odd
      )


writing-mode="from-page-master-region()"
reference-orientation="from-page-master-region()"

are both showing as errors?
