XSL FO is not specific to producing PDF output. The page size comes from the page-height and page-width property values, and for online display, the page could be either indefinitely long or indefinitely wide (though, in practice, formatting into pages is the norm). How an implementation goes about producing PDF would be beyond the scope of the spec.
