Schemas in Primer are not valid

Multiple of the schemas in the Primer are not valid.

Declarations are missing, but this is not noted. Readers trying to run 
or understand the schemas will therefore be stymied.

For example, s4.1 address.xsd does not have the declaration for 
UKPostCode. The comment

  <!-- simple type definition for UKPostcode -->
is not good enough, because it gives no indication that the user is expected to have learned
enough of XSD to fill in the gaps themselves.
