In the namespace fixup rules in 3.4.5.1 Attribute Default Value, rule 1 says:

If the [in-scope namespaces] of E contains a binding for N, no namespace fixup
is needed; the properties of E are not changed.

This is only true if the binding is a non-default binding, that is, if it binds
a specific prefix. A binding of the default namespace is no use.

I suggest changing "contains a binding for N" to "binds a prefix to N".
