This issue was originally reported by Asir Vedamuthu, Mary Holstege.
No intrinsic ordering or other distinguishable property for
annotations. Where annotations are promoted or otherwise coalesced
from multiple elements, there is no means to distinguish them, and
therefore reference them with a SCD.
