Bug in scope property of attributes (probably) and elements (maybe)

An attribute declaration may be local to an attribute group
definition, in which case its scope is specified as *absent*.

But when the group is referenced in a complex type definition, we
don't do anything to re-establish its scope as that ctd.

Probably true for elements/element groups as well, haven't checked in
