I think that http://www.w3.org/TR/xmlschema-1/#element-all shows that <all>
can have various groups and elements as children.  If I look at the Primer,
it says that <all> can only have elements as children and not groups.  What
is that about http://www.w3.org/TR/xmlschema-1/#element-all that I don't
understand?
