I also remember that we made a decision quite a while ago to add
<infault> & <outfault> under binding operation. It would be good this
decision be reflected in part one. Once part one is fixed, I will update
the primer accordingly.

I was reviewing the updates to Binding Operation and noticed the
following:
1. In 2.11.1 [1] shouldn't there be a {fault references} property? Add
the following text:
{fault references} OPTIONAL. A set of Binding Fault Reference components
2. In 2.11.2 [2] shouldn't there be <infault> and <outfault> elements?
