TEST: input form

I have created a test creation tool that should make it easier for other
people to create tests.
In particular it:
- creates the Manifest for you
- provides boiler plate
- checks RDF and XML syntax
- checks various style rules

then press

"Refresh from www.w3.org"

which updates the form from our test repository

and then press "Validate test"

which does some checking.

Then press "Zip up test" which returns a zip file with your test (complete
with Manifest).
