April CCXML: 2 bugs in 8_3_B.txml

1)

won't work to check the existance of the proper URI of dialog.vxml.

2) the <dialogterminate> results in both dialog.exit and 
conference.unjoined. The .txml has no
handler for conference.unjoined, which causes the catch-call transition 
to queue up another
user.PREPARE_NEW handler, triggering its transition to index past the 
end of assertions[] array.
