Updated ArchDoc LC issues list and annotated spec

Dear TAG,

I've been adding issues to the TAG's Last Call issues list [1].
There is a view of the issues by section of the spec [2].
DanC and I also wrote a script to generate an annotated view
of the spec with (pieces of issues) embedded [3]*. See also 
the list of reviewers [4] whose issues have already been
added to the issues list.

I have not yet updated the issues list with those issues
the TAG has already resolved; I intend to do that shortly.

* The ExIT TODO list includes this feature, and this is a useful
experiment.
